I certify that all conditions in the original permit are the same except property line distance has been corrected and other typographical errors and our casing program has been updated. Also, a new plat is attached because although SHL has not changed and lat/longs are the same as originally permitted, there was an error on the plat concerning the footages. There have been no other changes to land use, well construction or lease. This well is being re-filed under the old rules and does not require a Form 2A because this pad is built, there will be no expansion of the pad or additional surface disturbance, and this location is not in a RSO area.
