308A. COGCC Form 5. DRILLING COMPLETION REPORT

Within thirty (30) days of the setting of production casing, the plugging of a dry hole, the deepening or sidetracking of a well, or any time the wellbore configuration is changed, the operator shall transmit to the Director the Drilling Completion Report, Form 5, and two (2) copies of all logs run, be they mechanical, mud, or other, submitted as one (1) paper copy and, as available, one (1) digital LAS (log ASCII) formatted copy, or a format approved by the Director. Additionally, if drill stem tests, core analyses, or directional surveys are run, they shall be submitted at the same time and together with this completion report. All Sections 1 - 22 (if applicable) and the attachment checklist shall be completely filled out. The latitude and longitude coordinates in decimal degrees to an accuracy and precision of five (5) decimals of a degree using the North American Datum (NAD) of 1983 (e.g.; latitude 37.12345 N, longitude 104.45632 W), Position Dilution of Precision (PDOP) reading, instrument operator’s name and the date of the measurement of the “as drilled” well location shall be reported on the Form 5. If GPS technology is utilized to determine the latitude and longitude, all GPS data shall meet the requirements set forth in Rule 215. a. through h.

Within thirty (30) days of the suspension of commenced drilling activities prior to reaching total depth, the operator shall file a Drilling Completion Report, Form 5, notifying the Director of the date of such suspension of drilling activity stating the reason for suspension and the anticipated date and method of resumption of drilling, showing the details of all work performed to date. In cases of an uncompleted well, the initial Drilling Completion Report, Form 5, shall state "preliminary" at the top of the form. A supplementary Form 5 shall be submitted within thirty (30) days of reaching total depth.

How to become an designated agent:

There is no online registration for becoming an designated agent since a signature of the agent or employee is required to be kept on file at the COGCC. To become an agent with authorization to submit most eForm documents, you must complete a FORM 1A DESIGNATION OF AGENT (see Rule 302). Any party who acts as an agent for an operator in the State shall have filed an OGCC Form 1A with the OGCC. This designation shall remain in effect until terminated by letter from the operator.

If an agent has a Form1A on file with the commission but does not have access to eForm, the agent can contact the Colorado Operator User Administrator (COUA) for your company. You can look at the information that is currently in our database by going to the Database section and searching the Name, Address and Financial Assurance Tracking section.  The eForm administrator (COUA) for your company will be listed under the Employee List.

Instructions for Well Log Submittal via Electronic Forms

Accessing Logs

The best method to access the well logs is through the "Facilities" link on the "Database" page. You will need to know the API number, location, or well name to proceed. Once you have input the information to run the query you will be sent to a page with a list of wells meeting the criteria. Then click on "well" by the one that you are interested in. This will take you to the scout card displaying the well information. There are several icons near the top of the scout card page; select the magnifying glass or the word "DOCS" and it will take you to the scanned images of the well file. At the bottom of the list will be another index class titled "well logs" click on the log that you choose to view. It is recommended to download the logs to your hard disk and then open them. To do this right click on the log hyperlink for and select "save target as" then rename the file as all files are saved with the same name. This will allow you to view it without having to reload the file again if some error should occur. Errors may occur when viewing logs because of the large file size requiring a lot of memory in your computer to open and manipulate the files.

Viewing Logs

The  LAS formatted files as well as the scanned well logs in TIFF format require viewers that support these formats.

The LAS (Log Ascii Standard) format is an ASCII-type encoding that begins with a header and is followed columns of values describing specific measurements gathered at given depths. A free LAS file viewer can be found at the Kansas Geological Survey at the University of Kansas site.

The COGCC has a fairly complete set of scanned well logs in Colorado, logs prior to 1953 may not be present. These black and white document images are saved in TIFF file format with Group IV compression at 400 dpi. The TIFF file format offers the highest compression available for these documents, this makes them smaller to download but the are much larger when the viewer opens them. To view these documents, you must configure your WWW Browser to use a viewer that supports TIFF images and CCITT Group IV compression.
