Form 2 Attachment Guidance

 

1.  These documents provide guidelines for the preparation of attachments for the Application-for-Permit-to-Drill, Form 2. Each document includes the following information for the attachment:

1.    Definition: definition from COGCC Rule 100 Series, if applicable (only a small number of attachments are defined by a 100 Series rule)

2.    Rule/Policy Citation: language from the COGCC rule or policy that serves as the basis for the attachment requirements

3.    Purpose: description of the purpose the attachment serves as part of the permit application

4.    Requirements: identification of instances in which the attachment is required, followed by a list of minimum requirements for the attachment itself

5.    Common Errors and Issues: a list of the most common errors and issues that COGCC staff encounters with operators' submittals of the attachment

6.    Notes: any additional information relating to the attachment, including reference to other COGCC guidance documents

NOTE:

Every well is unique, with a specific set of parameters that dictate the regulatory requirements. This means it is not possible to create a "one-size-fits-all" guide for the preparation of the attachments for a Form 2. This document is not a comprehensive list of all potential requirements and errors; it focuses on the more common issues and errors.
